If you have loose skin on your tummy from pregnancies or dieting, you probably have given at least passing consideration to having a “tummy tuck.” Abdominoplasty is a common surgical procedure usually rendering excellent results – but all tummy tucks aren’t the same. Which one might be right for you?


There are three basic types abdominoplasty procedures – the mini, the standard and the extended. Which one you should consider can only be determined after a surgeon meets with and examines you. An abdominoplasty has a more extensive recovery time than many other cosmetic procedures, but it provides a long lasting and a very effective result. After the surgery, there is a truly dramatic, positive change to your body shape and your tummy will be flatter and tighter.


The simplest and easiest tummy tuck to recover from is the “mini” tummy tuck. This procedure is typically suited to women who have never had children, or have had just one pregnancy, and is appropriate if there is only a small amount of loose skin in the central lower abdomen. In this procedure the surgeon tightens the skin only and doesn’t repair the underlying abdominal muscle. Excess skin is excised from the abdomen and flanks and the underlying abdominal muscle is tightened as well. The scar runs across the upper pubic hair area and extends upwards toward the back on both sides. It is a long scar but many think the benefit is worth it, as it tightens the entire lower torso. Also, a really good surgeon is usually talented at leaving behind the bare minimum of scarring.


As in all medical/surgical procedures, your choices regarding abdominoplasty should be determined only after careful research and frank and open discussions with a licensed, qualified physician. Basic Information on Liposuction

Liposuction is a form of cosmetic surgery that aims to remove fat from certain parts of the body. The procedure is also known as Lipoplasty or Suction Lipectomy. Popular areas of the body, which this procedure has been used for, include the abdomen, thighs, arms and buttocks. As this is a surgical method of fat removal, it is NOT considered an alternative to diet and/or exercise. In fact the goal of liposuction is often referred to as “body contouring” as opposed to weight loss.

Since the 1980s, Liposuction has undergone many changes in order to minimize the risks involved. As such, there are a variety of techniques available, which may not require the use of a general anesthesia. Usually, a local anesthesia is preferred as it reduces the risks associated with a general one – besides any prolonged hospital stay.

Each technique has its pros and cons; factors can include the condition of the individual themselves and the physician. Generally speaking though, the individual should be in good health, an adult, have tried diet and exercise, yet still find a significant amount of fat left on their body.

Having diabetes, an infection, heart and/or circulation problems and/or taking any medication that affects the healing of wounds (anticoagulants) normally preclude one from the procedure. There are also other considerations that need to be accounted for before an individual undergoes the procedure.

Considerations include the technique used and type of anesthetic. The elasticity of the skin covering the potential area is also considered. If the skin is not sufficiently elastic, it may not tighten properly for the desired effect.

Avoid smoking for several weeks prior to the procedure. Smoking can affect blood circulation. Again, no anticoagulants should be taken as well. This can affect the healing process. Also, no alcoholic beverages should be consumed.

In terms of ensuring a safe and expeditious recovery, the individual should drink enough water to prevent dehydration. Again, avoid alcohol, avoid using any ice packs, heating pads or submerging the affected areas in water.

In terms of some of the techniques, they are the following: basic liposuction with a fluid injection, tumescent liposuction, use of stitches, super wet liposuction, ultrasound assisted liposuction (UAL), power assisted liposuction (PAL) and external ultrasound assisted liposuction (XUAL or EUAL). Some individuals may opt to undergo liposuction combined with some other procedure. However, it is commonly discouraged.

Techniques for cosmetic lengthening were described in 1990. More than 10,000 men have had such surgery, but reliable information about results and complications have not been published in a peer-reviewed journal (a journal where the quality and content of the research is checked by independent experts). Given the number of operations performed, this fact is both astounding and worrying.

Through surgery, the penis can be visually enlarged, usually by just over an inch. The surgeon will cut the ligaments that hold the penis in its usual position and this allows the penis to descend. Weights, or stretching devices, are then used for a few months to give a permanent increase in size. When this ligament is cut, the penis may look longer because more of it hangs outside the body. But cutting the suspensor ligament can cause an erect penis to wobble and position itself at odd angles, particularly when erect.

There are two techniques in common use for increasing penile girth:

- Injection of liposuctioned fat, from the abdominal wall or thighs, into the dartos fascia, under the skin of the penile shaft.

- Placing grafts of dermis (a layer of tissue from under the skin surface that is well supplied with blood vessels) and fat from the groin or buttock area within the penile shaft.

The results following injection of liposuctioned fat can be deeply disappointing. Up to 90 percent of the fat can disappear within a year. Grafts seem to do rather better, but there is no reliable published data on long-term results. At least one person has died from bleeding after augmentation surgery. Apart from the risks of infection and bleeding, specific complications exist for each procedure.

Lengthening Procedures:

- Scrotalisation of the penis, in which the penis appears to arise from the scrotum instead of the abdominal wall.

- “Dog-ears” at each end of the operation scar.

- Sloughing of the skin used to increase penile length (the skin has a poor blood supply and therefore dies, leaving a deep, ulcerated area).

Girth-increasing Procedures:

- Loss of girth due to fat re-absorption.

- Nodule formation, if the implanted fat does not disappear uniformly. The penis can be left with a deformed appearance, with irregular fatty lumps all over it.

So maybe you are better off without any surgery after all.

The next thing you can go for takes a non-surgical approach. The scam is widely known as “penis enlargement pills”. Penis enlargement pills cost a lot less than surgery, less than $50 per bottle, and if you will be using them for a year it will only amount to $600, compared to $4000 – the starting point for penis enhancement surgery.

Penis enlargement pills are herbal pills, so they don’t have to be tested. The marketers can make any claims whatsoever about their pills – I’ve even seen pills that can make you grow taller. The good news here is that you won’t experience bad side effects, if any. These pills are nothing more than vitamins with or without an addition of testosterone. You can get intoxicated on them, so don’t try to take too many at once. If you notice any unusual side effects (nausea, vomiting, headache, tension or pain in muscles and joints etc), call a Poison Control center right away.

The last method to try is penis enlargement devices such as stretchers. These are also non-expensive and some people build them at home. Trying any home-made penis enlargement device is a big no-no. There are a million risks involved here and you don’t want to risk injuries to your penis when you know surgery may not be a desirable option as well.

Approved stretchers and traction devices may work for some men, but they are not so widely used because exercising with them takes time. If you do have time they may prove an efficient and safe method for you, but always be sure you understand all the instructions before using any such device on your penis.

Along with the increase in popularity is the increase in concerns from the patients. The decision to undergo any type of elective surgery is extremely personal. They want to ensure how to select the most qualified plastic surgeon for their particular surgery.

 

 The Canadian Society for Aesthetic Plastic Surgery (CSAPS) and the Canadian Society of Plastic Surgeons (CSPS) encourage Canadians to educate themselves before their trip to the plastic surgeon’s office.

 “Most importantly, patients need to feel comfortable talking with their physician, before and after surgery and we hope that by raising awareness of available tools that Canadians can use, we will achieve that and discard the myths that exist  around plastic surgery.”

 

There are advertisements suggesting that breast implants have to be changed every 10 years and replaced with cohesive gel implants. We asked Dr. Lista and Dr. Warren their opinion on this confusing advertising message. We want to know why a woman would or should replace her implants, especially if she is absolutely okay.

 

“Any elective surgical procedure carries some risk. Problems and a higher rate of complications can incur when you introduce a second risk. There is no medical evidence to suggest that breast implants should be replaced if there are no problems.

 

 Plus, there is a huge misconception that one implant is better than another. There is a difference in implants and a good surgeon uses whatever implant is right for that particular patient.”  Dr. Lista further explains that he does not use just one supplier’s brand or style of implant. “It depends on the patient”.

 

Just as important as your choice of surgeon is the comfort to know that your surgeon has the knowledge of several options for your personal surgery.  We suggest that you ask your surgeon why he or she would chose a certain type of implant and what surgical technique is suggested for your own body type.

 

 “There are basically two types of breast implants: saline and silicone.  Saline implants can spring a leak and deflate – something which can only be corrected by having an operation to insert a new implant.  I would not recommend unnecessary surgery to replace saline implants when there is nothing wrong with them.

 

 However, older silicone implants like those banned in 1992 were often filled with a liquid form of silicone which could potentially leak and get into the surrounding tissue.  There have been two main changes to silicone implants: a more durable shell (thicker and stronger), and the gel inside is cohesive, which means that if a crack develops in the shell, the gel will stick together, and not leak out like a liquid filler would.” explained Dr. Warren

 

 

 

Obviously we would want to know for sure before we would replace breast implants, but,  is there a suggestion that women replace their cohesive gel implants, just in case?

 

Dr. Warren explained further that, “If the shell of a silicone gel filled implant is thought to be ruptured, most surgeons will recommend its replacement.  However, it is difficult to determine if such a shell has become broken.  Currently, the best means to detect a crack in the shell of a cohesive gel implant is with an MRI and that is not 100% accurate. Also, some surgeons feel that a cohesive gel implant with a crack in the shell may be left alone because the body creates a protective layer of scar tissue around any object.  This thin layer, which is similar to Saran Wrap, will contain the gel if there has been disruption of the implant shell.”

 

Another surgical procedure we wanted clarification on was liposuction and the mixed opinions surrounding tumescent liposuction (does not always mean without general anesthetic).

 

“Tumescent is a technique used in liposuction where large volumes of dilute local anesthetic are injected into the fat.  This method can be used with the patient awake or under general anesthetic. With local anesthetic there is a risk of xylocaine toxicity, because of the amount of xylocaine( local anesthetic) needed to inject while the patient is awake. General anesthetic has made significant advances; with new drugs and safety measures the patient is barely asleep.

 

Even with light anesthetic I ensure it is administered properly by an anesthetist with me during surgery to ensure safety and eliminating any toxicity concerns.”clarified Dr. Lista.

 

 Problems and dissatisfaction can be minimized by fully understanding and having realistic expectations. It is extremely important to choose a surgeon who will clearly and thoroughly explain potential side effects and risks. He or she should also have proper and extensive training, along with an accredited facility.

 

Dr. Warren explained, “Unless you are doing liposuction on a small section at a time, most plastic surgeons believe a better result can be achieved if the patient is  under general anesthetic. Certainly, if large or multiple areas of the body are to be done for a patient who is awake, it is difficult and potentially dangerous to achieve complete freezing of all areas because of the large amount local anesthetic which must be injected.

 

Based on the person’s weight, surgeons can calculate the maximum dosage of local anesthetic which can be given to any patient and determine if general anesthetic is in the patient’s best interest.”

 

A clear understanding of the qualifications of the surgeon you choose is imperative.  He or she should also have proper and extensive training, along with an accredited surgical facility if not operating at a hospital. Truth About Silicone Implants: Affordable Cosmetic Plastic Surgery in the Philippines

Numerous case reports in the 1980s appeared to suggest a linkage between silicone gel implants and connective tissue diseases and symptoms. The principal connective tissue disorders included scleroderma, rheumatoid arthritis, systemic lupus erythematosus, Sjogren syndrome, dermatomyositis, polymyositis, and polymyalgia rheumatica. There was also a concern over the possibility that the implants were linked to breast cancer and problems in breast-fed newborns. In response to this, the US-FDA in 1992 issued a moratorium on silicone breast implants and restricts the use of the implants for breast reconstruction and clinical studies only.

Subsequently, a number of large, population-based retrospective studies were conducted to test the association between silicone breast implants and connective tissue disorders. All studies have concluded that there is no association between silicone breast implants and any connective tissue disease. In addition, since the moratorium on silicone breast implants, several studies have proved that these devices are safe and do not cause connective tissue disease, malignancy, or risk to breast-feeding infants. Furthermore, clinical studies conducted by the British Department of Health and the Institute of Medicine of the National Academy of Sciences both demonstrate that silicone breast implants are safe for pregnancy, lactation, and breast-feeding.

In 1999, a landmark report, undertaken by the Institute of Medicine (IOM) concluded that silicone implants do not cause major health problems such lupus or rheumatoid arthritis. And while no one can ignore the fact that silicone – and saline – breast implants may cause localized problems for some patients, numerous scientific studies over the years have shown that silicone gel-filled implants are both safe and effective for breast augmentation and reconstruction. In fact, silicone gel breast implants are widely used in 60 countries around the world including Europe, and now are available for use in the U.S. market.

The U.S. Food & Drug Administration (FDA) announced November 17, 2006 the approval and availability of silicone gel-filled breast implants, ending a 14-year ban on the use of silicone implants for cosmetic breast surgery. Now all women over 22 years old seeking cosmetic breast augmentation and women seeking breast reconstruction surgery will be able to select silicone breast implants. The availability of silicone gel implants will create new options for women considering breast surgery for the first time and for those who have had previous surgeries but are seeking new replacements or revision.

Silicone breast implants offer several advantages over saline implants. They also have some disadvantages, so you and your surgeon should carefully weigh the pros and cons, if you are considering this option.

 

The major benefits of silicone breast implants are in the realm of a more natural look and feel. These implants are filled with silicone gel, which is softer and more natural (in the way it feels to the touch) than saline breast implants. They feel so soft and natural, that it is often impossible to discern them from natural breast tissue.

 

Rippling is far less likely to occur in women with silicone implants than in women with saline implants. This is because silicone gel is far more viscous (thicker) than saline, so it does not move or slosh, as saline can.
